In this section you will find three programs
1. PAD Questionnaire : is a questionnaire to print out and fill in in all its parts for each patient. A barcode corresponding to the test tubes must be placed in the appropriate space, on the first page of the questionnaire. YOU MUST SEND IT TOGETHER ALONG with the PATIENT’S T-TUBES, at the end of the study.
2. PADFORM DRUG CODE, is the file you should print out only once if/and where appropriate, it contains drug codes to complete the section CURRENT MEDICATION > Medication of the Questionnaire.
3. PADFORM DISEASES CODE, is the file you should print out only once if/and where appropriate, it contains the disease codes for the MEDICAL HISTORY section of the questionnaire.
